Understanding Social Determinants of Health

Social determinants of health are the conditions in which people are born, grow, live, work, and age. These include:

  • Housing stability and neighborhood safety
  • Employment and income
  • Education and access to health services
  • Food security
  • Transportation
  • Social support networks
  • Involvement with the justice system

For individuals struggling with addiction, especially those coming out of incarceration or homelessness, these factors can play a significant role in both addiction and relapse.

Integrated Behavioral Health Services
Our counselors help patients address trauma, anxiety, and depression. Mental health services—including medication when needed—are part of our full recovery model.
